Checklist of Items Needed to Develop Your Financial

  • Completed “Confidential Questionnaire” and “Living Expenses” worksheets
  • Latest paycheck stubs and bank statements
    -Provide an average annual pay increase rate (2%, 3%, etc.)
  • Annual Social Security benefits statements
  • All pertinent information regarding present investments such as stocks, bonds, mutual funds, certificates, annuities, real estate, etc.
  • Pension plans, 401(k) and other qualified plan statements (balances, holdings, interest rates, etc must be included)
  • Latest employee benefit statement booklets
  • Liability statements: including all mortgages, credit cards, auto loans, and other outstanding loans.
  • Insurance policies or summary of coverage statements (life, disability, long-term care, property/casualty, cash balances, interest rates, terms, etc.)
  • Income tax returns (past three years, including schedules)
  • Copies of legal documents (e.g., wills, trusts, prenuptial agreements, divorce decrees, etc.)
  • Information on unique financial events or situations (e.g., major purchases, expected inheritances, promotion, etc.)
  • If applicable, business financial statements and tax returns for last two years, articles of incorporation, buy/sell agreements, and group benefit employee package
  • All pertinent information regarding assets owned by dependent children or parents that will be used to meet goals (529 education plans, Coverdell, UGMA/UTMA, etc.)
  • List of financial goals with time frames and estimated amounts (today’s dollars)
    Examples:
    1. $2,500,000 for retirement in 10 years
    2. $120,000 for child’s education in 17 years
    3. $2,000 for annual vacation
